For starters immune response is extremely complex and usually vaccines are a degraded or completely sterilized version of the viruses they are supposed to vaccinate you against, mixed with something that forces your immune system to go into overdrive, sniff them out, kill them and create a bunch of defenses against them, not just antibodies.

Normally, vaccines take years to test and develop and that is when the virus in question has been studied for a while.

Back in the 1800s, rates of diabetes and obesity were lower, but also, life expectancies were lower. Whole foods, but rough lives, and poor medical technology. COVID-19's severity and infectiousness is highly age-stratified, with higher viral loads and more severe illness in older people. Wherever people have low life expectancies, it finds it very, very difficult to spread. Wherever there is a large elderly population, or a large number of people with metabolic syndrome, it spreads quickly and causes severe illness.
